INSTRUCTIONS FOR COMPLETING AGENCY SURVEY
Completing
the Agency Survey

Because the
Agency Survey requires the collection of technology data at the local
level, the printed “paper” form must be completed before attempting to
complete the survey on-line.
Additional resources, forms, and instructions such as an optional
“Computer Inventory Worksheet” in Excel may be downloaded directly from
the Resources page of this website.
To insure that the
survey reflects an accurate assessment of your agency’s technology, it
is suggested that Sections B2 to E6 of the survey be completed in
consultation with the Head Start program’s management team. This might
be accomplished by completing it jointly in a meeting. Another option
would be to have each management team member complete sections B2-E6 of
the agency survey individually. These individual surveys may then be
analyzed and used to complete the final Agency Survey that will be
completed on-line. Only one Agency Survey may be completed for
each agency.
TARGET DATE FOR
COMPLETION OF ALL SURVEYS IS JANUARY 15, 2007.
Reporting Head
Start Data
This survey should
be limited to reporting data on the Head Start program (i.e. Head Start
staff, locations, and resources). However, in organizations such as a
local education agency or community action agency that operates multiple
programs in addition to Head Start, the survey data should include
shared resources (i.e. technology resources, equipment, and software
that may not have been purchased by Head Start directly but are used
by the Head Start program or Head Start staff).
Delegate
Agencies
Completion of the
surveys by delegate agencies is optional. If the grantee wishes to
collect this survey information from its delegate(s), please forward the
Agency & Staff Survey forms to each delegate and ask them to complete
the Agency Survey and the Staff Surveys on-line in accordance with these
instructions. When completing the Agency Survey, delegates must select
the Grantee Name and then enter the grantee number and delegate ID for
their agency and the name of the delegate agency.
Survey Results
Survey results and
summaries will be available on-line through agency, state, and regional
reports.
Instructions
for Completing Agency Survey On-Line
Ø
When all items of the
“paper” Agency Survey are complete, please enter the Agency Survey
results on-line. Only one Agency Survey per agency.
Ø
Using your internet
browser, type in this address:
www.aacshs.org & hit the Enter key.
Ø
On the home page of this
site, click on “Surveys” (at the top of the page)
Ø
You will be prompted to
enter a user name and password. Enter the username and password
provided to you and click on “LOGON”
Ø
Click on “Agency Survey”
(at the top of the page)
Ø
Complete the Agency Survey
responding to each survey item.
Ø
After all items have been
completed, click on “Submit” at the bottom of the form and you will
receive a confirmation that the survey has been submitted

Survey is complete and you
may close your browser.
