AGENCY INSTRUCTIONS FOR COMPLETING STAFF
SURVEY

Each grantee should
have the following number of staff complete the Staff Survey directly
on-line and/or complete the attached “paper” survey which may then be
completed on-line by a data entry person. The agency may elect to
have all staff complete the survey; however, the agency should
insure that the following minimum numbers of Head Start staff complete
the survey:
- All
Head Start Administrators, Directors, Managers, Coordinators, and
Specialists
- Minimum of 10%
of all office clerical staff
- Minimum of 1 or
10% (whichever is larger) of the classroom teachers (or home-based
teachers) at each Head Start center or school selected at
random.
- Minimum of 10%
of all Family Advocates and Health Services Staff selected at random
but representative of multiple centers or locations
- Minimum of 5%
of all other classroom and non-classroom staff selected at random
but representative of multiple centers or locations.
Completing the
Staff Survey
The Staff Survey is
point & click and may be completed in five minutes or less. After
selecting the staff to complete the survey in accordance with the above
sample, the agency may use any or all of these methods to request staff
to complete the survey:
- Disseminate a
copy of the attached Staff Survey to selected staff (as paper copy
or email attachment) and instruct staff to go on-line to complete.
- Email the
instructions at the top of the Staff Survey form to selected staff
with internet access and ask them to go on-line to complete the
survey.
- For staff
without internet access, disseminate the paper copy of the form to
the selected staff; have them complete the survey; and then return
to the central office where you can have a data entry person enter
the survey(s) on-line.
TARGET DATE FOR
COMPLETION OF ALL SURVEYS IS JANUARY 15, 2007.

Results
The grantee may go
on-line at this web-site to monitor how many staff have completed the
survey. Survey results and summaries will be available on-line at this
site as well.
Instructions
for Completing Staff Survey On-Line
Ø
Using your internet
browser, type in this address:
www.aacshs.org & hit the Enter key.
Ø
On the home page of this
site, click on “Surveys” (at the top of the page)
Ø
You will be prompted to
enter a user name and password. Use the the username and password
provided to you and click on “LOGON”.
Ø
Click on “Staff Survey” (at
the top of the page)
Ø
Complete the survey
responding to each survey item.
Ø
After all items have been
completed, click on “Submit” at the bottom of the form and you will
receive a confirmation that the survey has been submitted
Ø
Survey is complete and you
may close your browser.
Ø
Note: If you do not
have access to the internet, you may complete the paper form and send
this completed form to your agency’s central office where it will be
entered on-line.
